Early Postoperative Intraocular Pressure Elevation afta Vitreoretinal Surgery

Objective: Di evaluation a intraocular pressure (IOP) afta vitreoretinal surgery is critical fi mek sure di normal function a di eye. Di purpose a dis study was fi determine di incidences an risk factors a early postoperative IOP elevation afta vitreoretinal surgery. Method: Data did collect an retrospectively analyze fram 150 patients (150 eyes) weh did get vitreoretinal surgery fram March 2012 to December 2012 inna Tongji Hospital. IOP did measure before surgery an pan day 1, 2, 3, 4-7 afta surgery by Goldmann applanation tonometer. Ocular hypertension was defined as IOP ≥ 24 mmHg. Di incidences an risk factors dem did analyze. Results: 87 a di 150 patients dem was man an 63 was female. Di IOP did elevate significantly inna 54 eyes (36.00%) widin 1 week afta vitreoretinal surgery. Among dem, 31 eyes (57.40%) did happen pan day 1; 14 eyes (25.93%) did happen pan day 2. Di incidences a elevated IOP between different primary diseases neva have no statistical difference (p>0.05). However, patients wid proliferative diabetic retinopathy (PDR), an rhegmatogenous retinal detachment (RRD) wid proliferative vitreoretinopathy (PVR) grade ≥ C2, did have higher rates a IOP elevation. Vitrectomy combined wid cataract surgery or scleral buckling neva have no significant difference inna IOP elevation compared wid vitrectomy only (p>0.05). Di rate a IOP elevation between 20G pars-plana vitrectomy (41.76%) an 23G pars-plana vitrectomy (27.40%) did have statistical difference (p=0.033). Di incidence a IOP elevation wid intraocular tamponade a C3F8 was significantly higher dan simple vitrectomy (chi2=7.723, p=0.005), while wid silicone oil, di difference neva significant (chi2=3.627, p>0.05). Conclusion: IOP measurement afta vitreoretinal surgery is important fi monitor an prevent unintentional high IOP as it a one common complication afta vitreoretinal surgery. Di risk factors a early IOP elevation include di 20G pars-plana vitrectomy an C3F8 injection. Early treatment a IOP may prevent IOP spike fi protect di vison.
